Submit inquiry and required documents​

  • ​Insurance eligibility check (in-network confirmation)

  • Submit for initial evaluation approval

  • Schedule evaluation (phone overview + in-person assessment)

  • Complete parent assessments (Vineland-3 & BASC-3)

  • Final insurance approval (may take 4–8 weeks)
